NEW PATIENT INFORMATION
Welcome! Are you in need of a new PCP? Great! We're here to help. We are affiliated with MDVIP and work with most insurance companies, offering services that surpass the norm. Please note that this program requires an annual membership fee, but you'll soon see how we stand out from traditional primary care offices.
MDVIP was originally established in 2000, the network now spans 44 states, comprising 1,100 primary care physicians dedicated to making a difference one patient at a time. Corrales Family Practice partnered with MDVIP in August 2012, a decision for which we are forever grateful. MDVIP enables Dr. Thal to practice in a way that fosters longevity, promotes a healthy patient-physician relationship, and facilitates earlier disease identification for life-saving diagnoses.

We are currently not accepting new patients however, we would love the opportunity to be your provider in the future. If you have additional questions, please click the button below. Please check back in early Summer of 2025.
If you're ready to begin, please fill out the medical record request form below. We would like to request records from any physicians you've seen in the last 2 to 3 years, including specialists, urgent care/emergency room, and primary care. The form is located below. Please note that each facility does require it's own form.
Once the form is filled out, please send it to kgonzales@cfpnm.com. We are eager to schedule your first appointment. Please provide your availability for the initial 1-hour establishment appointment. Once the appointment is scheduled, please enroll with MDVIP by clicking “get started today”. We are looking forward to meeting you!
